**Registration begins July 1, 2026 for the 2026-27 school year!

*ALL PHYSICAL EXAMS MUST BE UPLOADED TO YOUR ATHLETE'S REGISTRATION PROFILE - NO PAPER COPIES WILL BE ACCEPTED*

• Prior to the first practice, all student-athletes must complete the online registration and have a valid physical attached to their profile. 

1. Click here to register: Athletes must have a current physical/clearance form on file and complete online registration.
   1. Note: We have a new sports registration program. You will need to create a new account before you register your athlete for the first time. Please enter HSSD as the organization name. 
2. Printable Physical/WIAA Clearance Form
   • You will need to complete and return/upload the form titled "Clearance Form." The other three forms are for the doctor's use.
   • WIAA rules for completing physical:
     • A physical fitness exam is required by the WIAA no less than every other school year.  
     • Physical examination taken April 1 and thereafter is valid for the following two school years. Example: A physical dated April 1, 2026 or after is valid for the 2026-27 & 2027-28 school years.

4. Concussion and Cardiac Arrest Information
   • Law requires signature annually from parent and students. This is part of the online registration process.

5. HIPAA - Privacy Form required by Bellin Licensed Athletic Trainers 
   • All parents of students participating in sports must sign this form annually. This is part of the online registration process.

7. Pre-Season Meetings
   • Each sport holds a pre-season meeting. 
   • Please visit individual sport websites or contact the coach for date of pre-season meeting if not listed.
